We are seeking a committed and compassionate Chronic Health Worker to join our multidisciplinary health team. This role is essential in supporting Aboriginal individuals, families, and communities to manage chronic conditions, increase access to primary health services, and improve overall social, emotional and physical wellbeing. Working closely with clinical staff and community stakeholders, the Aboriginal Chronic Health Worker will provide culturally safe, person-centred care and contribute to the development of long-term health outcomes within Aboriginal communities.

Essential Selection Criteria
Certificate in Aboriginal Health Care, or actively working towards Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care, with AHPRA registration.
Sound understanding of primary health care and clinical knowledge of chronic diseases, with demonstrated case management experience.
Proven ability to plan, prioritise, and manage workloads with minimal supervision.
High-level communication and interpersonal skills.
Ability to work collaboratively with a wide range of health professionals and within a broader clinical team.
Moderate computer skills and ability to use clinical record systems.
Ability to work as part of a multidisciplinary team and occasionally outside normal working hours.
Cross-cultural sensitivity and an understanding of issues affecting Aboriginal communities.
Strong commitment to Aboriginal Social and Emotional Wellbeing and high-quality customer service.
High level of integrity, professionalism, and ethical practice.
Valid Working with Children Check (WA).
National Criminal History Check Certificate (Police Clearance – valid within the last 3 months).
Current C Class Driver Licence.
Current CPR Certificate.
